UCC data from the mass data subscription purchase agreement from January 1, 1995 to the present is available in a mass format. The modification of the financing status of the Uniform Commercial Code is intended for the termination, continuation and/or transfer of the changes made to the financing statement. UCC-11 (California) Information Request Form. This is a Californian form that can be used in the Uniform Commercial Code Secretary Of State. Most forms can be filled out on your computer, printed and submitted to the Ministry of Foreign Affairs for submission. Certain information fields that do not apply under Section 9 of the New York State Uniform Commercial Code have been completed as “Not Applicable.” NOTE: UCC documents submitted to the Department of State are public documents that must be disclosed under the Uniform Commercial Code. The Department must also provide copies of UCC documents upon request and by paying the applicable fee. Please do not include Social Security numbers on a UCC document submitted to this office. The Addendum to the Amendment to the Financing Statement of the Uniform Commercial Code is used to add additional information that is not included in the Amendment to the Financing Statement. The addendum to the financing statement is used for additional information not included in the initial filing of the financing statement of the Uniform Commercial Code. UCC Forms The State of Georgia only accepts the following forms for UCC submissions. (Cannot be completed online) The Cooperative Addendum on Uniform Funding Statements to the Commercial Code is used to provide contact information to the Registrar to contact the applicant. The model used in this e-filing system is similar, but not identical, to the current paper form.
Be careful when filling out the electronic template. As of March 21, 2007, an e-mail address is a required field.
